You got me curious and I had to check it out myself. It's both, actually. You measure kind of diagonally from the sole to the tip of the big toe. That length in cm is your shoe size. In this case, 250 it's 250cm so the shoe size is 250.

@chuck you're welcome! someone uploaded a few of these images just a couple of days ago and i was surprised that more hadn't been uploaded by the same artist. i also went on somewhat of an artist searching rampage to find this other guy on pixiv, bakugeki no G, so there should be a lot more images coming up from pixiv as soon as you mods approve of them : )
