I think I've uploaded this about her I don't know, I'll dig after this and bump it if I have but she's size-relative. She can be whatever she wants to be since she's just ectoplasm. THe more life force she consumes the more dense she can be, so the smaller she is the more dense she is but she can also get MASSIVE and less corporeal, and she can also feed on life force in it's various forms to get that density back. She's whatever size she feels like being and whatever she wants to be. When you see her in images like this, it's the face she has in life but she's known to pop up as all sorts of things like burgers, baseball bats, cake, angry cats, umbrellas, whatever she can become to trick people because she's a grade A mischief maker.

Because they're physical and not many are printed. There are reasons that this may not be able to be reprinted. Largely because it was created in the 80s/90s it seems, and the original printing files may not be around anymore or may not be updated for newer mass printing technology. Just like any comic. If they do get what it takes to reprint it, the original print will still be expensive because it's a rarity. There were only so many made. That's just how comics generally are and why there's a collecting aspect to it in the first place.

I think it's for the same reason that people make reasons that, while they may not be realistic, they allow for a general fantastical amount of "reasoning" when it comes to why a person has shrunken or grown. Some people do it for story purposes, other people just basically say "oh well it's because she's a super villain demoness" and so on. I mean, it's silly, the organ removal thing, but it's just something to shrug at more than anything else. For the same reason that the original poster said "that's not how it works" before the "reasoning" behind it came to light. TL;DR, it's a double edged sword and you can't win either way.

It's a slippery slope. At some point it's going to become trivial because of how many filters there are being added and people will want one for every little thing. Personally, I really dislike real life images. But I also don't complain about seeing them because I know other people like them. If seeing something like this really ruins your day then I don't really know what to say. I've seen some things that severely grossed me out but I just ignore it and move on to things I do enjoy. It seems silly to linger and complain just because you don't like something when other people do like it. That's how communities are, people are always going to have differing preferences.

Did some research. The OC is a draenei. However I almost wrote it off as a demon because the creator of the character made it with a design flaw. Draenei horns are supposed to be the same color as their skin, so it's an easy mistake to make. I'll fix the tags.

There are people that do enjoy it, that's good enough. If you don't like it you certainly don't have to look at it or comment on how much you don't like it. Some people enjoy just plain old PoV content. I have plenty of giantess/giant fans that get PoV images from me for that exact reason. That's why they get approved. Simple as dimples. Let people enjoy the content and look the other way if it isn't your thing.
